Informed Consent to Telehealth Services

By creating an account, completing an intake questionnaire, or otherwise requesting care through The Project Rx, you ("Patient," "you") acknowledge that you have read, understand, and consent to the terms below. If you do not agree to any part of this consent, do not use the Service.
1. What Telehealth Is
Telehealth is the delivery of healthcare services using interactive audio, video, secure messaging, store-and-forward review of intake forms and photographs, and other electronic communications between a patient and a licensed healthcare provider who are not in the same physical location. Through The Project Rx, your provider may evaluate your medical history, review images and lab results you submit, prescribe medications where clinically appropriate, order laboratory testing, and coordinate fulfillment with a licensed pharmacy.
2. Expected Benefits
- Improved access to licensed providers without the need to travel to a clinic.
- More efficient evaluation, follow-up, and prescription management.
- Discreet, written documentation of your treatment plan and provider communications.
- Coordinated delivery of medications, including compounded formulations where clinically indicated.
3. Possible Risks and Limitations
- Information transmitted may be insufficient (for example, due to poor image quality, incomplete history, or lack of in-person physical examination) to allow for appropriate medical decision making.
- Delays in evaluation or treatment may occur due to deficiencies or failures of equipment, connectivity, or third-party services.
- Although secure protocols are used, there is a small risk that protected health information could be intercepted or accessed by unauthorized persons.
- In rare cases, your provider may determine that the services you have requested cannot be safely provided via telehealth and that an in-person evaluation is required.
- Medications, including compounded medications, carry risks of side effects, drug interactions, allergic reactions, and other adverse events. Compounded medications are not reviewed by the U.S. Food and Drug Administration for safety, efficacy, or quality before being dispensed.
4. No Guarantee of Results
You understand that no specific result, outcome, or cure is guaranteed. Outcomes vary based on individual health factors, adherence to the treatment plan, and other variables outside the provider's control. Not everyone who applies will qualify for treatment.
5. Provider–Patient Relationship and Continuity of Care
The Project Rx is a technology platform; it is not a medical practice and does not itself practice medicine. Clinical services are provided by independently licensed healthcare professionals who are solely responsible for clinical decisions, including whether to prescribe any medication. Your provider may not be available 24/7. Telehealth services through The Project Rx are not a substitute for a primary care relationship, an emergency room, or hospital-based care.
6. Emergencies
Telehealth is not appropriate for medical emergencies. If you believe you are experiencing a medical emergency — including but not limited to chest pain, suicidal thoughts, severe allergic reaction, signs of stroke, severe bleeding, or any life-threatening condition — call 911 or go to the nearest emergency department immediately. Do not use the Service to seek emergency care.
7. Your Responsibilities
- Provide accurate, current, and complete medical, medication, and identification information.
- Disclose all relevant symptoms, conditions, allergies, and medications, including over-the-counter products and supplements.
- Follow the treatment plan, dosing instructions, lab work, and follow-up schedule provided by your provider.
- Promptly report any side effects, adverse reactions, or changes in your condition.
- Maintain the confidentiality of your account credentials and use the Service only for yourself.
8. Privacy and Recordkeeping
Your protected health information is handled in accordance with our HIPAA Notice of Privacy Practices and our Privacy Policy. Communications conducted through the Service may be recorded in your medical record. Sessions are not video-recorded without your express, separate authorization.
9. Eligibility and Jurisdiction
You must be at least 18 years old and physically located in a U.S. state where The Project Rx and its affiliated providers are licensed to deliver care at the time of each encounter. You agree to provide truthful information about your physical location for each visit. Some services may be unavailable in your state.
10. Right to Withdraw Consent
Your participation in telehealth is voluntary. You have the right to withhold or withdraw your consent to telehealth services at any time, without affecting your right to future care or treatment, by notifying us through our Contact page. Withdrawal will not apply retroactively to services already provided.
11. Acknowledgment
By proceeding with care through The Project Rx, you acknowledge that you have read this consent, that your questions have been answered to your satisfaction, and that you voluntarily consent to receive healthcare services via telehealth under the terms described above.
