Dosing Overview
Wegovy® is initiated at a low dose and titrated over 16 weeks to the target maintenance dose of 2.4 mg per week. This gradual escalation is critical for tolerability — the GI side effects of semaglutide are most pronounced at higher doses, and the titration schedule allows the body to adapt before reaching the full therapeutic dose.
The 16-week titration schedule is longer than that used for Ozempic®, reflecting the higher maintenance dose. Some patients may benefit from a slower titration pace — spending additional weeks at intermediate doses — if they experience significant GI side effects. This can be discussed with and managed by your prescribing provider.
Wegovy® is administered as a subcutaneous injection in the abdomen, thigh, or upper arm, rotating injection sites each week. It is injected on the same day each week. Pens are pre-filled and single-use, with a new pen required each week at the maintenance dose.

The clinical evidence for Wegovy® is anchored in the STEP (Semaglutide Treatment Effect in People with obesity) trial program, a series of four Phase 3 randomized controlled trials designed specifically to evaluate semaglutide 2.4 mg for weight management — distinct from the SUSTAIN program, which studied the lower-dose Ozempic® formulation for diabetes.
STEP 1 — the pivotal efficacy trial — enrolled 1,961 adults with obesity or overweight with at least one weight-related comorbidity (excluding type 2 diabetes). Participants receiving semaglutide 2.4 mg achieved an average weight reduction of approximately 14.9% over 68 weeks, compared to 2.4% with placebo. Nearly 70% of the semaglutide group lost ≥10% of body weight, and approximately one-third achieved ≥20% reduction.
STEP 2 evaluated semaglutide 2.4 mg specifically in adults with type 2 diabetes — demonstrating meaningful weight reduction even in this population that typically responds less robustly to weight management interventions.
STEP 4 assessed the consequences of treatment discontinuation, finding significant weight regain within one year of stopping the medication — reinforcing the understanding that obesity is a chronic condition requiring sustained management rather than a temporary intervention.
These findings establish Wegovy® as the most efficacious anti-obesity medication approved by the FDA at the time of its approval, and the SURMOUNT program for tirzepatide (Zepbound®) has since demonstrated even larger average weight reductions in the dual GIP/GLP-1 class.
Safety Information
The side effect profile of Wegovy® is qualitatively similar to Ozempic® — both contain semaglutide — but the higher dose used in Wegovy® is associated with a somewhat higher frequency and intensity of gastrointestinal side effects, particularly during dose escalation. The 16-week titration schedule is specifically designed to minimize the impact of these effects by allowing gradual adaptation.
In the STEP 1 trial, approximately 44% of participants on semaglutide 2.4 mg reported nausea compared to 16% on placebo. The majority of GI events were reported as mild to moderate and diminished over time. Discontinuation rates due to side effects were approximately 7% in the semaglutide group — meaning that the large majority of patients were able to continue treatment.
Practical strategies for managing gastrointestinal side effects include: eating smaller, more frequent meals; avoiding high-fat, fried, or spicy foods particularly early in treatment; staying well hydrated; eating slowly and stopping when full; and taking the injection at a consistent time each week.
