Ozempic® (semaglutide) is an injectable GLP-1 receptor agonist manufactured by Novo Nordisk. What Is Ozempic®?

Ozempic® is the brand name for semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist available as a once-weekly subcutaneous injection. It was first approved by the U.S. Food and Drug Administration (FDA) in December 2017 for the treatment of type 2 diabetes mellitus in adults, and has since received additional indication for cardiovascular risk reduction in patients with type 2 diabetes and established heart disease.

Semaglutide belongs to a class of medications known as incretin mimetics. Incretins are hormones released by the gut in response to food intake; they signal the pancreas to release insulin and help regulate blood sugar. GLP-1 is one of the primary incretin hormones, and semaglutide is a synthetic analog designed to bind to and activate GLP-1 receptors throughout the body — but with a significantly longer half-life than the natural hormone.

The medication is administered via a prefilled injection pen once weekly, on the same day each week. It is available in three pen configurations delivering doses of 0.25 mg, 0.5 mg, 1 mg, and 2 mg — with patients typically starting at the lowest dose and titrating upward over several weeks under physician supervision.

While Ozempic® is FDA-indicated for type 2 diabetes, licensed providers may evaluate patients for other uses based on clinical judgment and individual health profile. A separate formulation of semaglutide — Wegovy® — is FDA-approved specifically for chronic weight management at the higher 2.4 mg dose.

The pharmacological durability of semaglutide is one of its defining characteristics. Its molecular structure has been modified to resist degradation by the enzyme DPP-4 and to bind reversibly to albumin in the bloodstream, extending its half-life to approximately one week — enabling the convenient once-weekly dosing schedule that has contributed to its broad clinical adoption.

Mechanism of Action

How Ozempic® Works

Semaglutide works by mimicking the action of the body's naturally occurring GLP-1 hormone — but with a crucial difference: where endogenous GLP-1 is broken down within minutes, semaglutide remains active in the bloodstream for approximately seven days, providing continuous receptor stimulation throughout the week with a single weekly injection.

GLP-1 receptors are found throughout the body — in the pancreas, brain, gastrointestinal tract, heart, kidneys, and liver — and activating them produces a coordinated set of metabolic effects. In the pancreas, GLP-1 receptor stimulation promotes glucose-dependent insulin secretion (insulin is released when blood glucose is elevated, but not when it is already low, which helps minimize hypoglycemia risk). It also suppresses glucagon secretion, a hormone that would otherwise raise blood glucose by triggering the liver to release stored glycogen.

In the gastrointestinal tract, semaglutide slows gastric emptying — the rate at which the stomach empties its contents into the small intestine. This slower transit time extends the period of satiety after meals, contributing to reduced overall caloric intake. Patients often report feeling full more quickly and less interested in food between meals.

In the brain, semaglutide acts on hypothalamic areas involved in appetite and energy regulation. GLP-1 receptors in the arcuate nucleus and other satiety centers respond to semaglutide signaling by reducing appetite and food-seeking behavior. This central effect is considered a key driver of the weight changes observed in clinical trials.

The combination of peripheral metabolic effects and central appetite modulation creates a multi-pronged mechanism that can influence body weight, glycemic control, and cardiovascular markers — making semaglutide one of the most pharmacologically complex medications in the metabolic space.

Dosing Overview

Titration Schedule

Ozempic® is initiated at a low dose and titrated upward over several weeks to improve tolerability, particularly with respect to gastrointestinal side effects. The standard titration schedule is designed to allow patients' bodies to adjust gradually to the medication's effects on gastric motility and appetite.

The starting dose of 0.25 mg is considered a therapeutic introduction rather than a clinically effective dose for glycemic control — its purpose is acclimation. Most patients begin to experience meaningful glycemic and appetite effects once they reach the 0.5 mg or 1 mg dose levels.

Your prescribing provider will determine the appropriate titration pace based on your response and tolerability. Not all patients advance to the maximum 2 mg dose — many achieve satisfactory outcomes at 0.5 mg or 1 mg, and dose decisions should always be made in consultation with your licensed provider.

The injection is administered subcutaneously (beneath the skin) in the abdomen, thigh, or upper arm, rotating injection sites weekly. Ozempic® pens are pre-filled and come with easy-to-use needles included in the kit.

The evidence base for semaglutide is extensive, built on a comprehensive clinical trial program called SUSTAIN (Semaglutide Unabated Sustainability in Treatment of Type 2 Diabetes). The SUSTAIN trials enrolled thousands of patients across multiple countries and established semaglutide's efficacy for glycemic control, weight reduction, and cardiovascular risk.

SUSTAIN-6, the landmark cardiovascular outcomes trial, demonstrated that semaglutide significantly reduced the risk of major adverse cardiovascular events (MACE) — including non-fatal myocardial infarction, non-fatal stroke, and cardiovascular death — in adults with type 2 diabetes at elevated cardiovascular risk. This cardiovascular benefit is part of the FDA-approved indication for Ozempic® in appropriate patients.

Across the SUSTAIN program, semaglutide consistently demonstrated superior HbA1c reductions compared to placebo and several active comparators, including other GLP-1 agents. Weight reductions observed across the trials ranged from approximately 4 kg to 6 kg on average, with higher doses and longer durations associated with greater effect.

Side Effects & Risks

Like all medications, Ozempic® carries a risk of side effects. The most commonly reported side effects are gastrointestinal in nature and tend to be most pronounced during the initial weeks of treatment and following dose increases. For the majority of patients, these effects are mild to moderate and diminish as the body adjusts to the medication.

Nausea is the most frequently reported side effect, affecting a significant proportion of patients particularly during dose escalation. Slower titration — spending more time at lower doses before advancing — can help reduce the severity of gastrointestinal symptoms. Eating smaller meals, avoiding high-fat foods, and staying well hydrated are commonly recommended strategies for managing GI side effects.

Weeks 1–4

Initiation & Adjustment

The 0.25 mg starting dose is an acclimation phase. Some patients experience mild nausea or reduced appetite. Glycemic effects are limited at this dose. Focus on adherence and documenting any side effects.
